The SAIL form
The license itself is the SAIL — Simple Agreement for Instant License v1.0 (MusicAtlas, CC BY-ND 4.0). Five fields, e-signatures accepted. The terms are also published machine-readable at the /license endpoint, for agents that read before the humans sign.
